Law and Order: Sheltered Episode

This episode was on yesterday afternoon and it contains the famous scene toward the end where the camera pans across the court room and Elizabeth Rohm's character is sitting at the counsel table, with 1 shoe off and 1 on. She has her right leg crossed over with her shoe on and the left foot, on the ground, with her black pump clearly a foot away.
I would really love to know the story behind what happened. Did she take her shoe off in between takes casue her foot hurt and just forgot to put it back on? Did she think she could just take the shoe off because it would be viewable??
I love stuff like this, that's not part of the scene, not in the script... You gotta wonder how many times this has happened and we just havent caught it. There is a movie called "Civil Action" with Travolta in it and the opening credits, just as they break away, there is a crowded bar scene and a woman is starting to dip. (she's not a character in the movie, just a crowded scene)
Can anyone think of others?
"Dr. T and the Women", I thought would be a good one as it's filmed a lot in Dallas, in shopping malls where women routinly dress up to shop, thought maybe there'd be something in the background. Maybe on the show "Friends", with Jennifer Aniston dressed up a lot in heels. "Johnson Family Vaction" has a background quick dip I believe inside an Indian Casino!
